About The Position

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse (RN) or physician in a healthcare, long-term care, or assisted living facility. The LPN is responsible for monitoring patients’ health, administering basic nursing care, and ensuring that patients’ medical needs are met. This role is essential in promoting the health, comfort, and well-being of residents by delivering high-quality nursing care in a compassionate and professional manner.

Requirements

  • Completion of an accredited LPN or LVN program.
  • Current and active Licensed Practical Nurse (LPN) license.
  • Strong clinical and patient care skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work well as part of a team and collaborate with other healthcare professionals.
  • Compassionate and patient-focused approach to care.
  • Proficient in electronic medical records (EMR) systems and nursing documentation.

Nice To Haves

  • Previous experience in a healthcare or long-term care setting.

Responsibilities

  • Provide direct basic nursing care to residents, including medication administration, wound care, and assistance with personal hygiene, feeding, dressing, and mobility.
  • Monitor residents' vital signs and health status, reporting any abnormalities or changes to the supervising nurse or physician.
  • Administer medications as prescribed by physicians, ensuring accurate documentation and monitoring for adverse reactions.
  • Maintain accurate and up-to-date documentation of patient care, vital signs, treatments, and changes in condition, ensuring compliance with facility policies and healthcare regulations.
  • Collaborate and communicate effectively with RNs, physicians, other healthcare professionals, residents, and their families regarding care plans and health updates.
  • Participate in care conferences and interdisciplinary team meetings.
  • Adhere to infection control protocols and maintain a clean, safe environment, using personal protective equipment (PPE) and reporting safety hazards.
  • Act as an advocate for residents, ensuring their needs are met, dignity is respected, and providing emotional support to residents and families.
