Responsibilities

  • Administering medications, monitoring responses, and reporting concerns promptly to the supervising clinician.
  • Performing routine assessments, vital signs, wound care, and daily living support to maintain residents’ comfort and safety.
  • Assisting with activities of daily living, mobility, and rehabilitation activities designed to promote independence.
  • Contributing to care planning, documenting changes, and collaborating with RNs, therapists, and family members to optimize each resident’s trajectory.
  • Ensuring infection control practices, safety protocols, and regulatory compliance are consistently followed.
  • Providing education and support to residents and families, addressing questions and easing transitions between care levels.
