About The Position

The Licensed Practical Nurse Under the direction and supervision of the Clinic Manager or Triage Nurse, uses and practices basic assessment, participates in planning, executes interventions in accordance with the plan of care in the PACE clinic and in participant’s homes, and contributes to the evaluation of individualized interventions related to the care plan. Administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to Senior Community Care participants and caregivers in regards to plan of care, nursing treatments or medications. Requirements

  • Graduate of an accredited school of nursing required
  • Current Practical Nurses License (L.P.N.)
  • Cardiopulmonary Resuscitation (CPR) certification required
  • A minimum of one year’s experience in working with the frail or elderly population required.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Responsibilities

  • Completes basic assessments and assists PACE RNs as delegated to assist in completing initial and semi[1]annual assessments of new PACE enrollees in the PACE clinic and in participant’s homes.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Performs nursing care in the PACE clinic and in participant’s homes per the LPN scope of practice in the state in which they are practicing.
  • Performs the administration of prescribed medications and treatments in accordance with nursing standards.
  • Participates in the nursing on-call rotation.
  • Maintains participants’ medical record and fulfills Senior Community Care charting and reporting requirements per policy. 7. Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians
