About The Position
MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs, enabling them to receive quality healthcare while remaining active in the community and living in their own homes. These programs are staffed by qualified, experienced licensed nurses and caregivers who offer individual assistance. This LPN/LVN role focuses on providing care, training, and support to individuals with brain injuries, developmental disabilities, or behavioral health needs. Requirements
- Current LPN/LVN license in MA
- Valid driver’s license and auto insurance
- Strong communication and teamwork skills
Nice To Haves
- At least 1 year of nursing experience preferred
Responsibilities
- Monitor health conditions and administer medications
- Provide treatments and personal care services
- Complete assessments and update care plans
- Train staff on safety and infection control
- Communicate with families, providers, and community partners
