Role Snapshot
Role: Licensed Practical Nurse (Home Health)
Location: Virginia Beach & Norfolk, VA • Onsite (home visits, local travel only)
Pay: $48 per visit (~$70,000/year avg.)
Schedule: Monday–Friday, 8am–5pm • 5–6 visits/day
Work with a stable, Medicare-certified home health team serving Hampton Roads. Clear routes and consistent workload.
What You’ll Do
Conduct home health visits and basic assessments
Deliver wound care, med administration, skilled nursing
Support start-of-care and follow-up visits (no OASIS lead)
Coordinate with RN case managers and therapists
Complete visit notes within 72 hours
Communicate changes in patient condition promptly

Requirements

• Active LPN license in Virginia (good standing)
• 6+ months of home health experience
• Current CPR certification
• Valid driver’s license and reliable insured vehicle
• Must pass OIG, state, and national background checks
