The Licensed Practical Nurse works as an integral member of the team providing services to senior residents who are aging in place or are clients in the Mental Health Clinics/PROS who are diagnosed with a behavioral health concern. The Licensed Practical Nurse is responsible for providing a range of wellness services to meet the behavioral health, medical, and care issues affecting those living independently within the Housing buildings and clients attending the Mental Health Clinics, Care Coordination or PROS programs. The Licensed Practical Nurse will be responsible for developing and implementing practices and protocols for clients ranging the entire life span. Residents in crisis or with behavioral health needs (such as hoarding, depression, anxiety, substance use disorders) will be reached out to, engaged and treated by the Licensed Practical Nurse. Additionally, the LPN will address and assess for medical concerns presented by clients in housing and the clinics/PROS. The professional will develop an individualized plan to address needs and include family, significant others and healthcare providers whenever possible. The primary goal of this employee is to support residents to maintain permanent affordable housing or, where appropriate, aide in transitioning to more appropriate housing. Within the Mental Health Clinics/PROS and Care Coordination, the primary goal is to assess medical concerns, perform health monitoring services, and provide linkages to treatment, in addition to providing health teaching and counseling around current and future health concerns.
