Under the supervision of the Director and Assistant Director of Nursing, the Licensed Practical Nurse plays a vital role in coordinating and delivering high-quality nursing care for geriatric residents.   • Assist in assessing resident needs and report clinical changes to the Registered Nurse.
  • Participate in planning individualized Nursing Care Plans.
  • Provide care related to activities of daily living and document clearly and accurately.
  • Deliver therapeutic care and evaluate patient responses.
  • Support the development of short- and long-term goals, including teaching and discharge planning.
  • Administer medications safely and accurately in accordance with the “five rights.”
  • Follow all Department of Nursing policies, procedures, and regulatory guidelines.
  • Provide resident care related to activities of daily living.
  • Support admission and discharge processes.
  • Assist with physical exams, diagnostic procedures, specimen collection, and therapeutic treatments.
  • Deliver postmortem care when needed.
  • Support nutritional needs including meals, supplements, and tube feedings.
  • Apply infection control and isolation procedures.
  • Initiate CPR and assist during emergencies.
  • Maintain timely, clear, and concise nursing documentation.
  • Assist with revising and updating care plans and treatment plans.
  • Participate actively in interdisciplinary and team care conferences.
  • Support and collaborate with the healthcare team.
  • Participate in training new personnel and reinforcing care plans with residents and families.
  • Help establish care priorities and assign tasks to CNAs.
  • Serve as charge nurse when Director or Assistant Director of Nursing is unavailable.
  • Assist with staffing and scheduling needs within the department.
  • Engage in ongoing quality improvement initiatives, committees, and in-service training.
  • Meet continuing education expectations, including six hours of annual in-services.
  • Attend at least 75% of staff meetings and demonstrate a commitment to ongoing professional growth.
  • Maintain excellent attendance and adhere to Wesley Woods Senior Living policies.
  • Demonstrate professional behavior and uphold Standards of Conduct.
  • Complete all required trainings, health assessments, and safety protocols annually.
  • High school diploma or equivalent.
  • Active Georgia Licensed Practical Nurse (LPN) licensure.
  • Current BCLS certification.
  • Ability to lift, turn, and assist residents safely — independently and with support/equipment.
  • Strong communication and interpersonal skills to engage professionally with residents and families.
  • Ability to stand and walk for long periods; frequent bending and stooping required.
  • Visual, hearing, and speech abilities necessary for documentation and resident communication.
  • Minimum of two years LPN experience preferred.
