Envision Unlimited is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to support our community living programs across Aurora, Kankakee, and Tinley Park, IL. In this dynamic, hands-on role, you’ll play a critical part in promoting the health, safety, and well-being of the individuals we serve. Working under the supervision of a Registered Nurse, you’ll conduct onsite assessments, administer medications, monitor treatment plans, support hospitalizations, and collaborate with interdisciplinary teams to ensure high-quality care. The Licensed Practical Nurse (LPN) at Envision Unlimited plays a vital role in assessing and addressing the medical, health, and safety needs of clients within assigned community living program(s). Under the supervision of a licensed Registered Nurse, the LPN conducts client assessments, documents medical needs and interventions, monitors medication regimens, and collaborates with internal and external partners to ensure client well-being.

Requirements

  • Current Illinois license as a Licensed Practical Nurse (LPN).
  • Valid CPR certification.
  • Valid driver’s license and access to a reliable vehicle.
  • At least two years of clinical nursing experience, preferably including one year working with individuals with developmental disabilities.

Responsibilities

  • Perform onsite client assessments, intervention and providing treatment
  • Administers medications including oral, topical and by injection
  • Perform onsite quality assessment of medication carts
  • Document client medical needs and interventions, including progress notes.
  • Monitor medication regimens; review medication records, physician orders, bubble packs, and MARs.
  • Review client medical records monthly or more frequently as needed.
  • Communicate effectively with Program Directors, CLS Administrators, the Division Director, physicians, and other medical personnel.
  • Support client hospitalizations, including admission, discharge planning, and hospital visits as needed.
  • Travel to multiple CILA locations each shift
  • Provide staff development and training related to health, safety, and medical topics.
  • Participate in agency in-service programs.
  • Serve on division or agency-wide committees as assigned.
  • Perform other duties as assigned.
