Under the direct supervision of a registered nurse (RN), the licensed practical nurse (LPN), within scope of the LPN practice and regulatory standards, assists the RN in providing and coordinating holistic care of the patient through honoring patients’ mind, body and spirit while respecting human dignity and the uniqueness of each individual. LPN clinical proficiency is based on a sound knowledge base together with the understanding of the theories and science of practical nursing practice. The LPN must have the ability to delegate appropriate work to unlicensed assistive personnel (UAP) while taking responsibility for the work delegated. The LPN has knowledge, nurse training and skill sets specific to populations of people with mental health and substance use diagnoses. Performs direct patient care and related tasks as outlined by scope of practice and including but not limited to: Assists RN with assessment of patient’s clinical and functional status. Performs assessment of patient’s clinical and functional status with input and oversight of an RN. Contributes to completion of patient database and ongoing assessment. Considers the patient’s culture, age and developmental level. Documents concurrently. Identifies patient risk factors. Performs physical and psychosocial assessment according to scope of practice. Collects physiologic monitoring data (patient weight, vital signs, lab results and diagnostic test results) upon admission and ongoing.
