About The Position

The Licensed Practical Nurse is responsible for the management of medication system of the clients and monitoring the general health of clients.

Requirements

  • NYS Licensed Practical Nurse is required
  • Current First Aide/CPR Certification
  • Minimum two (2) years post-licensure experienced providing nursing care in a clinic setting, or social service setting is required
  • Excellent analytical, evaluative, and human service management skills

  • Advanced level of computer proficiency with proficiency in Word, Excel, and the ability to learn new technologies and databases

Responsibilities

  • Conduct Health Screens for all new clients during shelter intake process.
  • Use motivational interviewing, recovery and trauma-informed approach when delivering services to clients.
  • Administer medication and document appropriately in Medical Administration Record (MAR).
  • Ensure documentation in MAR is accurate.
  • Ensure providers are aware of medications refills needed for clients on a timely basis.
  • Ensure pharmacy is informed and receive prescriptions for medication refills.
  • Ensure expired or redundant medications are removed from the medication cart.
  • Respond to emergencies and ensures follow-up with emergency services.
  • Collaborate with the PNP in coordinating follow-up care.
  • Educate clients on medication and treatment compliance strategies.
  • Communicate to the PNP all client medial concerns and assist PNP in determining medical care needs and coordination of care.
  • Monitor vitals and clients’ response to medications and side effects.
  • Review with PNP findings and assist with follow-up medical care as needed.
  • Order medical supplies as needed and stores supplies and medication records stored in a secured closet.
  • Secure medication cart in a locked secured location.
  • Participate in team meetings, case conferences, staff meetings and trainings, etc.
  • Collaborate with social service team in assessing clients’ needs, developing service plan and coordinating care for clients.
  • Document delivery of services, clients progress, etc. in electronic case record in accordance with agency policies and procedures.
  • Contributes to the overall success of the organization by performing all other duties and responsibilities as assigned by supervisor or designee, and being able to pivot with ever-changing priorities and newly added job-related responsibilities that arise based on business changes, needs, and demands.
