About The Position

This is a paraprofessional position as a Licensed Practical Nurse that assists in providing services to clients in the Healthy Start Program for Holmes & Washington Counties as needed. Provides services to Healthy Start prenatal and infant clients based on Healthy Start Standards and Guidelines (HSSG), Chipola Healthy Start Coalition (CHSC) System of Care and Well Family System (WFS) Dosages, inclusive of initial assessment, Healthy Start Care Coordination and referring clients to local agencies. Inputs prenatal and infant screens into the Department of Health, Health Management System (HMS). Maintains an individualized plan of care and Family Support Plan as necessary. Provides education and support services to include screening and intervention pathway tools as follows: Partners for a Healthy Baby (PHB); Breastfeeding Education and Support; Child Passenger Car Seat Safety Education; Safe Sleep Education; Tobacco Use Screening; Smoking Cessation and Reduction in Pregnancy Treatment (SCRIPT); Screening, Brief Intervention, and Referral Treatment (SBIRT); Intimate Partner Violence (IPV) Edinburgh Depression Scales; Show Your Love/One Key Question (SYL/OKQ) Interconception Services; Ages and Stages Questionnaire (ASQ3); and Mother and Babies Stress Management Education. Identifies and evaluates client's problems and makes recommendations to the Program Coordinator. Participate in regular team meetings. Coordinates client services and education utilizing home visits, telephone contacts, office, and clinic visits. Provides follow up with local agencies to assure that necessary services are met. Reports to Program Coordinator when follow-up with client is necessary. Maintains documentation of services provided in the client's record. Enters all services and documentation into the Well Family System (WFS). Refers clients for direct support services such as family planning, well-child checkups and immunizations. Will teach the following safety classes: Safe Sleep, Car Seat, and any other class Healthy Start offers to clients. Responsible for inventory and issuances of Healthy Start items to include but not limited to car seats, pack-n-plays, diapers, bottles, pacifiers, etc. Will inventory, store/retrieve, or issue Healthy Start items/documents to or from storage unit or inside/upstairs of CHD. Will lift and carry items up to 45 lbs. to clients’ vehicles or homes. Will participate in community outreach events. Provides Presumptive Eligibility for Pregnant Women and Simplified Eligibility for Pregnant Women (PEPW), completion of appropriate forms to assure that clients are covered by Medicaid.  Gives information on Family Planning Medicaid Waiver and Economic Self Sufficiency Programs (SNAP, Medicaid, and Cash Assistance). Participates in educational training and staff meetings.  Obtains training for professional development and administrative knowledge of program requirements.  Attend workshops, meetings, and in-services to maintain license and advance knowledge level through TRAIN, LMS, and other mandatory educations sources. Serves as Information Custodian for Healthy Start Records assigned responsibility for maintaining information in accordance with state and federal statutes, rules, regulations, and DOH policy. Performs other duties as assigned by supervisor.

  • High school diploma or equivalent
  • Valid driver’s license
  • Push, pull, lift, or carry items up to 45 lbs. & walk up and down stairs
