About The Position

The Licensed Practical Nurse (LPN) plays a crucial role in ensuring high-quality healthcare services for residents. Under the supervision of the Registered Nurse (RN), the LPN provides direct nursing care, provides case management, clinical consultation and supports staff in implementing and enforcing healthcare training and protocols. This role requires strong clinical skills, leadership abilities, and a commitment to promoting resident well-being and independence.

Requirements

  • Must be licensed by the Massachusetts Board of Registration in Nursing to practice as an LPN in the Commonwealth.
  • Experience: Minimum one years of nursing experience in a healthcare facility, school, or community setting.
  • Legal & Compliance: Must authorize Criminal History Background Check (CORI) and release information from DPPC, DDS, and other regulatory agencies.
  • Training: Successful completion of all state and agency-mandated training and certifications, as assigned by CRC
  • CPR/AED, first aid
  • Physical Ability: Capable of performing essential job duties, with reasonable accommodations as needed.
  • Driver’s License & Transportation: Valid Massachusetts driver’s license and access to reliable transportation.
  • Technical Skills: Proficient in Microsoft Office applications and electronic health records (EHR).
  • Understanding of behavior modification techniques.
  • Fluent in English with the ability to read and interpret important documents (e.g., safety rules, procedure manuals).
  • Strong written and verbal communication skills.
  • Ability to effectively communicate with individuals receiving services, staff, families, and community members.
  • Capable of presenting information clearly in both individual and group settings.
  • Significant endurance required for job-related physical activities.
  • Regular activities include: sitting, standing, walking, stooping, kneeling, and crouching.
  • Frequent use of hands/fingers for handling tools and reaching.
  • Must be able to generate force to maneuver up to 100 pounds (assistance provided when needed).
  • Ability to lift up to: 30 pounds from floor to waist, 10-15 pounds from waist to overhead
  • Ability to carry out detailed written or oral instructions.

Responsibilities

  • Enforce medical/nursing policies and procedures to ensure consistent application across programs.
  • Ensure compliance with best practices, state, and federal requirements.
  • Conduct periodic audits of health care record-keeping.
  • Develop individualized person-centered medical guidelines in collaboration with healthcare providers.
  • Ensure proper implementation of health care recommendations and screening by conducting audits and training program staff.
  • Collect and analyze health-related data to assess compliance and identify areas for improvement.
  • Organize and participate in interdisciplinary Red flag meetings, DDS/DPH provider meetings, etc, to review the health management of individuals within CRC.
  • Provide nursing consultation to program management, supervisory staff, individuals, and families.
  • Serve as a resource and advocate for health care access, ensuring individuals receive appropriate medical care.
  • Manage case coordination for individuals with complex or acute healthcare needs.
  • Attend medical appointments and case conferences when complex health issues arise.
  • Conduct on-site health assessments and treatments as needed.
  • Develop and maintain relationships with healthcare providers to facilitate quality care.
  • Stay updated on third-party insurance, Medicare, and Medicaid regulations to assist in coverage and benefits navigation.
  • Participate in discharge planning for individuals transitioning into CRC residential homes.
  • Provide reinforcement Medication Administration Certification (MAP) specific training under the supervision and guidance of an RN
  • Ensure compliance with MAP and Department of Public Health (DPH) regulations.
  • Stay current with DDS, DPH, and MAP regulations to maintain CRC’s compliance.
  • Conduct annual and ongoing training on infection control practices, person-centered medical guidelines, vital signs, modified diets, specialized training on emergency medical interventions (ex: epi-pens, seizure protocols), and new diagnoses as they arise.
  • Participate in new staff orientation to educate on health-related practices.
  • Collaborate with other CRC departments to ensure individuals’ health, safety, and well-being.
  • Provide emergency first aid and accompany individuals to emergency medical services as needed.
  • Participate in weekly phone on-call rotation and provide in-person nurse coverage as needed.
  • Work with DDS and DPH health care personnel to refine community service health policies.
  • Assist in ordering and managing health care equipment and supplies.
