Licensed Practical Nurse (CT LPN)
About The Position
Sadler Health Center is recruiting a full-time Licensed Practical Nurse to be part of their Medical Care Team. The role involves working Monday-Friday from 8 am to 5 pm, with no weekends or late evenings. As a Sadler Health Licensed Practical Nurse, you will be part of a unique, team-oriented care model that provides comprehensive medical, dental, and behavioral health services, as well as primary care and case management services to patients. This is a hands-on position in a fast-paced clinic, offering the opportunity to assist with patient visits and varied procedures, caring for patients of all ages. The center's mission is to advance the health of the community by providing inclusive, high quality and compassionate care.
Requirements
- Active Pennsylvania License as a Practical Nurse
- Certificate or degree from an accredited program
Responsibilities
- Conduct med reconciliations
- Assist with specialty referrals
- Assist with pre-authorizations
- Ensure continuity of care as part of family medicine practice
- Assist with patient visits and procedures
- Care for patients of all ages
