This Licensed Practical Nurse (LPN) position is within the CHOC Southern Clinics, specifically the Pediatric Multispecialty Clinic in Franklin. This clinic offers young patients access to a variety of specialists, lab testing, x-ray services, and speech, occupational, and physical therapy in a convenient location away from the main hospital. The LPN supports licensed healthcare team members by providing direct patient care, offering healthcare information to patients and families, and adhering to established plans of care. The role involves close collaboration with Registered Nurses (RNs) or other licensed healthcare team members to evaluate patient care plans and progress towards goals, with all patient care activities performed under the delegation and supervision of a licensed professional in accordance with VUMC standards and licensure regulations. The shift is Monday-Friday, 8:00 a.m. - 4:30 p.m., with no holidays, weekends, or on-call duties.

Requirements

  • LIC-Licensed Practical Nurse - Tennessee certification
  • 1 year of relevant work experience
  • Graduate of an approved discipline specific program
  • Novice proficiency in Clinical Patient Documentation, demonstrating sufficient fundamental proficiency to successfully demonstrate patient documentation in practical applications of moderate difficulty, generally working under direction.
  • Novice proficiency in LPN Patient Education, demonstrating sufficient fundamental proficiency to successfully demonstrate patient education in practical applications of moderate difficulty, generally working under direction.
  • Novice proficiency in LPN Care Planning, demonstrating sufficient fundamental proficiency to successfully participate in the development of the plan of care and organize work, generally working under direction.
  • Novice proficiency in Clinical Competency, demonstrating sufficient fundamental proficiency to successfully demonstrate nursing interventions in practical applications of moderate difficulty, generally working under direction.

Responsibilities

  • Support licensed healthcare team members in the provision of patient care, including direct patient care.
  • Provide healthcare information to patients/families.
  • Follow established plans of care.
  • Work in close collaboration with the RN or a licensed healthcare team member to evaluate the patient's plan of care and progress toward goal attainment.
  • Undertake all patient care activities with the delegation/supervision of a licensed professional in accordance with VUMC standards and licensure regulations.
  • Collaborate with patients/families and health care providers in the delivery of health care services.
  • Partner with the healthcare team by following established plan of care to ensure patient progress for effective outcomes.
  • Support plans to promote health and a safe and healthy environment.
  • Contribute to the nursing assessment by collecting, reporting, and recording objective and subjective data in an accurate and timely manner.
  • Participate in the development of the plan of care in consultation with the licensed healthcare team.
  • Anticipate resource needs, identify distinct tasks, set priorities, schedule activities, meet deadlines, and organize work within areas of direct responsibility.
  • Handle multiple assignments often simultaneously, prioritizing work into manageable and measurable units.
  • Adapt to changes to patient care and area needs with minimal disruption and loss of productivity.
  • Follow up to assure problems and issues are resolved.
  • Demonstrate nursing interventions such as wound care, point of care testing, and vital signs.
