For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. STATEMENT OF THE JOB The Licensed Practical Nurse works as an integral member of the team providing services at agency locations across a range of service lines. The Licensed Practical Nurse is responsible for providing wellness services and health monitoring to meet the behavioral health and care issues affecting those served by the agency’s Integrated Health and Wellness programs. The primary goal of health monitoring is to identify clients at risk due to poor physical health and medical conditions, improve their health outcomes through early identification and care, and prevent lowering their quality of life or early death. Health monitoring goals include: providing health assessments and health monitoring to a wide range of agency clients, proving monitoring services to high risk clients weekly, with the aim of reducing medical hospitalizations. The Licensed Practical Nurse will be responsible for developing and implementing practices and protocols specific to the population served. Clients in crisis or with behavioral health needs (such as hoarding, depression, anxiety, substance use disorders) will be reached out to, engaged and treated by the Licensed Practical Nurse. The LPN will develop an individualized plan to address needs and include family, significant others and healthcare providers whenever possible. The primary goal of this employee is to support clients to monitor and maintain primary health goals. The Licensed Practical Nurse is part of the team and assists team members in understanding the needs of clients and help improve their quality of life.
