Requirements
- Must be 18 years of age or older
- Must have an active LPN license in the state of Indiana
Nice To Haves
- At least one-year of related experience is preferred
Responsibilities
- Provide direct care to residents by monitoring their condition, administering medications, and performing necessary tasks such as taking vital signs and documenting care
- Collaborate with the interdisciplinary team to develop and implement care plans that enhance the physical, social, and mental well-being of residents, ensuring that all care is compliant with regulatory standards
- Assist with staff training, participate in quality assurance activities, and maintain clear communication with residents, families, and medical staff about changes in care
