Licensed Practical Nurse | Ancillary Support Staff

Nova Patient CareBailey’s Crossroads, VA
$18 - $23

About The Position

Are you a Licensed Practical Nurse who thrives in fast-paced clinical environments and genuinely loves making a difference in patients' lives? NOVA Patient Care is looking for a compassionate, detail-driven LPN to join our Virginia-based clinic team in a dual-function role that blends hands-on clinical support with meaningful care coordination. As ancillary support staff in our clinics, you'll be the backbone of daily patient flow — rooming patients, recording vitals, maintaining EHR records, supporting providers during exams, collecting lab specimens, and ensuring a clean, compliant, and efficient clinical environment. On the care coordination side, you'll manage our Chronic Care Management (CCM) program — enrolling eligible patients, building individualized care plans, conducting monthly check-ins, tracking CCM minutes for billing and supporting Remote Patient Monitoring (RPM) activities. This is not a routine nursing role. It's a chance to own your patient relationships and be a central point of continuity in their care journey. If you're ready to step into a role where your clinical skills and care coordination instincts both get to shine, we'd love to hear from you.
