Hendee Road House Hendee Road Home is a 6 bed residential program for men with severe mental illness and are discharging from long term hospitalization. The program will address the full range of daily living needs of each individual with a focus on increasing independence and self-efficacy of individuals through 24-hour/7-day a week staff support in the residence and the community. Staff interventions focus on teaching skills around daily living, symptom management, coping skills, medication management, health & nutrition, healthy relationships, personal safety, money management, substance use education, linking to recovery supports, and providing sober activities. Staff will also provide crisis intervention in attempt to reduce need for hospitalizations and emergency room visits. Licensed Practical Nurse Duties & Responsibilities Conducts Medical Assessments (a client self-report on medical history), LOCUS, Co-occurring Screens and Functional Assessments. Support individuals with participating in their medical appointments or procedures, obtaining updates vitals quarterly, and assisting clients with obtaining lab work as ordered. Collaborates with individuals and the treatment to contribute to individualized, person centered treatment plans. Provides individual and group education around self-care, medical conditions, mental illness, and medications. Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions. Develop crisis plans for clients and work with other staff to respond to crisis situations as needed Provide medication administration for injectable medications and other medication monitoring and coordination needs. Collaborating with Primary Care, Hospitals, Visiting Nurses and Specialty Medical Providers in the community. Provide education, support and coaching around skills of coordinating self-care and medical/behavioral health care needs. Provide skills teaching, such as assisting clients in planning and preparing nutritional meals including diet and nutrition, Diabetes education, illness prevention, engage and communicate with family and friends and safety within the community, Provide intensive skill building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and using community resources Additional duties as required
