Position Overview Reporting to the Home Care Director/Manager, the Granite VNA Licensed Practical Nurse (LPN) delivers age appropriate care to the patient in the home setting utilizing standard nursing best practice: implementation of plan of care as established by a Registered Nurse, and within scope of practice defined by the NH Board of Nursing. LPNs will effectively interact with patients, care givers and other healthcare team members while maintaining all standards of professional nursing. LPNs are supervised by the Registered Nurse assigned to the patient.
