The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. This role involves demonstrating knowledge of nursing principles, competency in nursing expertise, and utilizing age and population-specific standards related to the physical and psychosocial needs of the resident as per care plan. The LPN will perform testing to evaluate data reflective of the resident's status and provide necessary care, as well as recognize problems, recommend solutions, and respond effectively in emergent situations. Key responsibilities include ensuring the health, comfort, and safety of residents, contributing to assessments, administering medications or treatments, and providing technical support to healthcare professionals. The position requires thorough documentation of resident interactions, outcomes related to care, medication, behavior, activity, and dietary. The LPN must be skilled in establishing and maintaining effective working relationships with residents, healthcare providers, and the public, and be able to work effectively as a member of an interdisciplinary care team. Clear communication and maintaining confidentiality of sensitive information are essential. The LPN will also counsel residents, family members, and/or resident advocates on prevention and treatment plans under the direction of a registered nurse, advanced practice provider, or physician, and apply knowledge of resident education principles to develop self-care programs. Travel is required to support multiple locations within a 150-mile radius of the permanent address.
