Requirements

  • Current NY State LPN License required
  • Positive, dependable, and caring with a team-oriented mindset
  • Ability to provide high-quality care while following established procedures and safety standards
  • Strong communication, documentation, and leadership skills

Responsibilities

  • Coordinate resident care and monitor overall wellness
  • Oversee and participate in medication administration and treatments
  • Respond to changes in condition, incidents, and daily needs on the floor
  • Serve as a coach and supervisor for HHAs, PCAs, and Medication Technicians
  • Communicate with families and the interdisciplinary team to ensure seamless care and follow-through
  • Support a home-like environment where independence and dignity are prioritized
  • Administer medications, injections, and treatments with precision and care
  • Perform ongoing resident assessments and report changes in condition
  • Provide direct nursing care in partnership with CNAs
  • Support short-term rehab patients as they recover and return home
  • Maintain accurate documentation and collaborate with nurses, therapy, and physicians
