Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Requirements

  • A minimum of one year of LPN experience (Hospice experience is preferred but not required).
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and as part of a collaborative team.
  • Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations.

Nice To Haves

  • Hospice experience is preferred but not required

Responsibilities

  • Monitoring vital signs
  • Assisting with wound care and personal care
