Licensed Practical Nurse - Rehab Care Center - Part Time or Full Time
About The Position
Under the direction and supervision of an RN/GN, the LPN/GPN is expected to utilize his/her knowledge of practical nursing, age-specific needs, the care/treatment of the Rehab patients. The LPN/GPN is to be an active member of the interdisciplinary treatment team and work collaboratively in the development, implementation and revision of each patient’s interdisciplinary treatment plan. The LPN/GPN is to observe and evaluate each patient, document his/her observations and evaluations and provide the RN with relevant clinical information.
Requirements
- Must be a graduate of an accredited LPN program.
- A current license as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
- Must be BLS certified required (or obtain within one month of hire).
- A commitment to service excellence and performance improvement.
- Must possess nursing knowledge of medical, surgical and behavioral health nursing to effectively provide direct patient care, observe and document outcomes.
- Provide patient and family education.
- Excellent writing, computer and interpersonal skills.
- The ability to work collaboratively as a member of the interdisciplinary team.
Nice To Haves
- Prefer 1 year of medical surgical experience preferred
