About The Position

Community Care Hospice is currently seeking a dedicated and skilled Licensed Practical Nurse (LPN) to provide high-quality hospice care in a home setting. This position plays a key role in supplementing the nursing care provided by the RN and ensures the delivery of patient-centered, end-of-life care with dignity, empathy, and professionalism.

Requirements

• Current LPN license in the state of Kansas.
• Minimum of one year experience with chronically ill and/or terminally ill patients.
• Valid Kansas Driver’s License and proof of auto liability insurance.
• Reliable transportation and an acceptable Motor Vehicle Record per company standards.

Responsibilities

• Deliver direct patient care under the guidance of a Registered Nurse and in alignment with the patient’s plan of care.
• Monitor, report, and document changes in patient condition; collaborate with the interdisciplinary team.
• Educate patients, families, and caregivers on care techniques and safety measures.
• Assist with personal care activities including hygiene, mobility, and basic home maintenance.
