About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. Full-time: Spring Hill & surrounding areas The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of a team to give direct nursing care to patients requiring intermittent visits. The LPN collaborates patient care with the case managing RN, physician, and patient and follows a patient specific treatment plan.

Requirements

  • Graduate of state-approved school with course in pharmacology
  • Current license in the State of Pennsylvania
  • Minimum of two [2] years experience in patient care
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state

Responsibilities

  • Provides skilled nursing care that includes activities such as teaching and treatments (including IV therapy- if IV certified, venipuncture and complex wound care) and health promotion practice
  • Executes preventive, rehabilitation and restorative nursing procedures appropriate for the patient’s care and safety
  • Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, changes in patient’s physical, mental or emotional condition to the physician and the supervising Registered Nurse
  • Evaluates the patient and home situation to determine the health teaching required at each skilled visit.
  • Instructs and educates the patient and the designated representative regarding the care requirements of the patient in the home. Conducts a comprehensive physical examination and nursing assessment of the patient at each skilled visit.
  • Tablet is synced within 24 hours.
  • Updates the medications listed in the electronic health record on an ongoing basis
  • Addresses all areas of the skilled nurses note at each visit, including a comprehensive pain assessment as needed
  • Documents all verbal orders obtained from the physician as they are received
  • Completes a hospital hold coordination note as soon as he/she is aware of the transfer
  • Documents all communications with the physician and the interdisciplinary team on the patient’s electronic health record.
