About The Position

Here, you’ll work in a supportive and collaborative environment with a team of dedicated professionals. We provide the tools and resources needed to foster meaningful connections with our residents and deliver outstanding care. A Typical Day: Participate in and document admissions and transfers. Visit each resident and instruct staff of needs to provide optimum care. Interact with staff, residents, families, and physicians to provide optimum care for each resident. Observe and document any changes in resident’s mental or physical condition and communicate same to director and physician. Respect the rights and privacy of each resident while fulfilling responsibilities for their care. Administer medications according to established procedures and in a timely manner. Participate with facility Quality Improvement as directed, as well as chart audits. Work independently with proper direction. Perform all duties assigned in an effective, timely, and professional manner. Check fax and Matrix Care throughout shift for new orders. Complete assigned monthly summaries on time. Utilize LPN calendar and treatment log.

Requirements

  • Must be licensed by the state of South Carolina and in good standing with a current active license.
  • Must have the ability and knowledge to act in a supervisory capacity.
  • BLS certified OR willingness to get certified within 3 months of employment.
  • Consistently demonstrates professionalism and clinical skills.
  • Maintain professional appearance and conduct exhibiting a respectful and courteous demeanor at all times.
  • Ability to stand, stoop, bend, reach, squat, climb, reach above head, carry, and lift up to 50 lbs.
  • Ability to communicate clearly and effectively both verbally and written.
