Requirements

  • Experience in a memory care community is highly desirable.
  • Prior long-term care experience is highly desirable.
  • CDP preferred.
  • Excellent communication and interpersonal skills are required.
  • Active RI Nursing License (LPN or RN) is required.
  • Minimum of 3 years Nursing experience.

Responsibilities

  • Read Daily Report at the beginning of shifts.
  • Supervises nursing care with particular attention to ill Residents.
  • Takes vital signs on residents as needed.
  • Notes change in condition in residents and notify physician and medical POA.
  • Requisitions supplies and equipment.
  • Orders medications according to facility routine.
  • Recommends changes in resident care when necessary and communicate this to the Director of Wellness.
  • Charts all pertinent information in resident's chart in a timely manner.
  • Call Physicians and Pharmacies as needed to report discrepancies in orders.
  • Provides professional leadership to all nursing personnel.
  • Informs residents of any changes in their care.
  • Administers medication and treatments as ordered by the physician and charts appropriately when needed.
  • Assists nurses' aides and med techs as necessary.
  • Fills out incident reports when resident, visitor or employee is injured.
  • Advises employees of facility policy regarding employee injury and proper medical attention.
  • Notifies the Director of Wellness of all incidents in a timely manner.
  • Fills out interagency referral forms.
  • Admits, discharges and transfers residents.
  • Follows through assigning care for new residents.
  • Uses universal infection control precautions and ensures use by nursing personnel.
  • Provide our residents with daily care in accordance with our established policies and procedures and assure that the highest degree of quality care will be always maintained.
