About The Position

The Ashtabula County VA Clinic offers general medical care, including primary care, behavioral health, diabetes care, laboratory services, nutrition services, and more. The City of Ashtabula is located on Lake Erie's coastline about an hour between Cleveland, Ohio and Erie, Pennsylvania.

Responsibilities

Explains procedures to patients in understandable language, considering age and gender needs.
Ensures the environment aligns with patient-centered care vision.
Maintains self-control, responds consistently to work requirements, and demonstrates a positive attitude toward reassignment.
Responds promptly to patient calls, ensuring customer service focuses on all interactions.
Communicates respectfully and works effectively with PACT, administrative staff, patients, and visitors.
Provides emotional support to patients and families to help them adjust to health changes.
Demonstrates knowledge and skills for complex nursing assignments in various care settings.
Completes non-standard and specialized nursing activities, resolving a range of nursing problems.
Accurately reports and records significant patient information, demonstrating good judgment in applying procedures.
Prepares and administers medications, documenting accurately in CPRS.
Recognizes and responds to emergent situations, reporting promptly to the RN or provider.
Applies knowledge of human behavior to work effectively with patients and staff.
Provides teaching and encourages participation in activities of daily living (ADLs), and patient self-care.
Performs complex treatments and procedures according to policy, collects specimens for lab analysis.
Sets up and performs treatments requiring auxiliary equipment.
Adheres to Safe Patient Handling standards and policies.
Prepares patients for tests, examinations, and treatments.
Applies assistive/protective devices as needed.
Documents observations and care provided following established policy.
Observes, identifies, and responds to patient needs, providing education considering physical, emotional, cultural, spiritual, socio-economic, and age-related factors.
Contributes to the PACT team in developing, implementing, and evaluating treatment plans, promoting patient participation.
Provides competent care under adverse conditions, ensuring safety for self and others.
Functions as a team leader under RN supervision, conducting group meetings for patients.
Serves responsibly as a nursing team member, interacting with patients, families, and care providers effectively.
Follows environmental policies and procedures, ensuring compliance with regulations, promoting environmental stewardship through pollution prevention, waste minimization, and resource conservation.
