This position delivers care to patients and assists the Registered Nurse in monitoring, observing, and reporting reactions to treatments. The role participates in the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. It involves effectively interacting with patients, significant others, and other health team members while maintaining high standards of professional nursing. The LPN provides high-quality care to various patient populations including Geriatric, Medical/Surgical, Adult, Pediatric, Obstetric, and Telemetry. This position will also involve working as a CNA in the unit. Key duties include assessing and monitoring patient condition, documenting and communicating results to physicians, conferring with RNs regarding abnormal/critical lab values, and administering prescribed medications, IV fluids, and enteral feedings. The LPN will also answer patient calls, assemble equipment, perform procedures and treatments, assist medical staff, provide basic patient care (e.g., dressing wounds, preventing pressure ulcers, catheterizations), prepare patients for examinations, collect samples, evaluate nursing intervention outcomes, assist patients with daily living activities, measure vital signs, and provide patient and family education. The role requires working as part of a healthcare team, participating in performance improvement activities, delegating care to certified nursing assistants and patient service associates, adhering to safety and evidence-based practices, prioritizing assignments, coordinating nursing care, and accessing resources for patient care planning.
