About The Position

Hawthorne Foundation Inc. is seeking a Licensed Practical Nurse (LPN) for a group home for individuals with developmental disabilities. The position is full-time, requiring flexibility to meet program needs. The LPN will work across five Individualized Residential Alternatives (IRAs) and will be responsible for various medical and health-related tasks, ensuring compliance with OPWDD regulations and maintaining health records.

Requirements

  • Certification by the Department of Education, State of New York as a Licensed Practical Nurse.
  • Knowledge and experience in the field of developmental disabilities working with individuals over 21.
  • Sensitivity to the needs of individuals, their families, and personnel.
  • Ability to relate to others, especially those with special needs.
  • Good communication skills (verbal, written, and telephone).
  • Computer skills.

Responsibilities

  • Attend medical appointments with individuals.
  • Organize and maintain medicine rooms.
  • Scan and upload documents into Therap.
  • Assist Head Nurse in retaining, reviewing, and maintaining individuals' health and immunization records.
  • Ensure acquisition of physical examinations by physicians under the guidance of the Head Registered Nurse.
  • Maintain complete and current medical documentation for each individual.
  • Physically assess individuals to recognize normal and abnormal variations.
  • Participate in team conferences and individual program planning.
  • Document changes in medical status under the guidance of the Head Nurse.
  • Recognize how cultural lifestyle, family dynamics, and economic factors affect health.
  • Understand nutritional needs based on dietary assessments.
  • Provide first aid to individuals as needed.
  • Assist in counseling and teaching individuals, families, and staff on personal hygiene, family life, dental care, sex education, and safety education.

Benefits

  • Sign-on bonus of $1500 for new full-time employees, paid in two installments: $750 after 3 months and $750 after 6 months.
