About The Position

The Licensed Practical Nurse (LPN) plays a vital role in delivering compassionate, patient-centered care as part of Unity's dedicated healthcare team. Under the guidance of a Registered Nurse, the LPN provides direct patient care, supports the development and execution of care plans, and performs delegated nursing tasks to ensure the highest quality of service. This position is primarily based at our Meng Residence and will also provide support in the community as needed, assisting patients in their homes or at partner facilities.

Requirements

  • Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin
  • Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage

Responsibilities

  • Provide basic nursing care
  • Record nursing care given and report to the appropriate team member changes in the condition of a patient
  • Assist with the development and revision of a nursing care plan
  • Reinforce the teaching provided by a registered nurse and provide basic health care instruction
  • Perform delegated nursing acts beyond basic nursing care under the direct supervision of a registered nurse
  • Monitor IV fluids on controlled infusion pumps as directed by a registered nurse
  • Maintain level of competency in required areas
  • Comply with all applicable local, state and federal laws and regulations as well as policies and procedures of Unity
  • Satisfy mandatory training requirements regarding local, state and federal regulations and/or Unity’s policies and procedures
  • Consistently and concisely document activity according to department protocols and standards
  • Promote and maintain confidentiality
  • Maintain personal and professional boundaries
  • Perform other job-related duties as assigned
  • Maintain regular and reliable attendance
  • Shares in providing seven days-a-week coverage
