About The Position

The Licensed Practical Nurse provides safe and individualized nursing care to patients in the Ambulatory Care area which includes, Specialty Care settings. He/she interacts, collaborates, and communicates effectively with the veteran, families, caregivers, professionals and other supportive staff both internal and external. He/she initiates appropriate actions based on his/her observations of the patient's condition. The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. The LPN/LVN also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the veteran. The LPN/LVN performs within this functional statement under guidance of the Nurse Manager or designee. Assigned tasks require basic technical competence and skill. He/she requires a minimum of supervision and performs the functions listed below consistently with skill and effectiveness. Duties are performed under the general supervision of a RN or Healthcare Provider (HCP). Individuals at this grade are expected to have broad working knowledge of practice nursing procedures, however, completion of more complex practices or procedures may be subject to closer high-level review. The LPN/LVN works in collaboration with the Health Care Provider, Registered Nurse (RN) and the Medical Support Assistant (MSA). Together, with the patient and their caregiver support system, they work to manage the care of a panel of adult patients across the continuum. Additional duties include but are not limited too:

Responsibilities

  • Demonstrates proficiency in initiating, performing and completing assigned duties while providing care to diverse patient populations.
  • Observes, identifies, and responds to patient needs for medication, medical equipment, and patient/family education while considering emotional, cultural, spiritual, socio-economic and age-related factors.
  • Establishes constructive and positive relationships with patients/families in order to promote open communication and socialization skills, taking into consideration human behavior, patient motivators and reactions to situations.
  • Identifies patient's educational needs to include equipment, medication, treatment procedures, and diagnosis. Assists with teaching patient/family as appropriate.
  • Demonstrates flexibility by willing and promptly accepting changes in work assignment and locations.
  • Organize work, set priorities, delegate tasks/responsibilities and follow-up for completeness in order to provide and expedite patient care.
  • Knowledge of how to apply Prevention and Management of Disruptive behavior techniques to resolve issues before they reach escalation.
  • Assess life threatening conditions and initiates and/or assists with CPR or other appropriate interventions including prompt notification of an RN or MD/DO to provide appropriate direction and intervention. Must maintain a current Basic Life Support Certification.
  • Responsible for the daily management of the Provider's scheduled appointments. This is accomplished through assisting Provider with preventative care and/or chronic disease management, obtaining vital signs, offering patient education, completes screening questions for a variety of preventative and/or chronic disease measures, manages immunizations injections and all other ordered medications and assists with exams/procedures (pelvic exams, joint injection, wound care, EKG's, Phlebotomy, bladder scans).
  • Other responsibilities include assisting the PACT team or other Interdisciplinary team members with panel management such as follow-up of lab and test results, patient education and care coordination. Is an active participant in planning, implementation and evaluation of care.
