By integrating in-person and virtual primary care with urgent care, we aim to deliver personalized, hands-on care that proactively educates, connects patients with resources, and simplifies the complexities of downstream care. Join us as a Licensed Practical Nurse (LPN) and be part of this innovative approach to redefining healthcare. Your Impact Patient Care & Triage: Conduct comprehensive patient triage by taking vital signs, documenting medical history, and performing observational examinations to identify immediate healthcare needs. Assist with patient exams, ensuring all necessary equipment and documentation are prepared, while supporting providers during testing and procedures. Clinical Procedures: Perform diagnostic and testing procedures, including vision, hearing, drug screens, breath alcohol testing, spirometry, and more. Execute point-of-care testing such as strep, flu, RSV, SARS-CoV-2, glucose, lipid panel, and urinalysis. Provide cardiac and airway management, including ECGs, AED use, nebulizer treatments, and oxygen administration. Deliver wound care assessments, immunizations, and medication administration per federal and state regulations. Patient Engagement & Documentation: Assist with patient inquiries, communicate test results, and explain medication or therapy plans. Maintain accurate electronic medical records, documenting patient histories and treatment details. Observe and report changes in patients' conditions, taking necessary action as needed. Clinic Operations: Ensure medical instruments and equipment are clean, sterilized, and maintained per organizational policies. Maintain a stocked and sterile workspace, supporting smooth clinic operations. Collaboration with Clinical Team: Work closely with other members of the clinical team to ensure seamless coordination of patient care. Communicate effectively and efficiently with providers and other healthcare professionals to relay patient information and updates. Welcome and support the training of new team clinic team members.
