About The Position

The Licensed Practical Nurse (LPN) in the Patient Aligned Care Team (PACT) is responsible for providing ongoing, continuous care to a panel of Veterans. This role involves utilizing various tools for effective patient identification and intervention, managing clinic workflow, and facilitating safe transitions of care. The LPN will engage with Veterans and their families, provide health education, and ensure the delivery of safe patient care through adherence to established procedures and protocols.

Requirements

  • U.S. Citizenship is required.
  • Graduate of an approved school of practical or vocational nursing.
  • Full, active, current, and unrestricted licensure as an LPN in the U.S.
  • Proficient in written and spoken English.
  • Ability to pass a pre-employment physical evaluation.
  • Participation in seasonal influenza and COVID-19 vaccination programs is required.

  • 1 year of experience in a primary care or outpatient setting.

Responsibilities

  • Provide ongoing, continuous care to assigned panel of Veterans.
  • Utilize registries and tools for effective identification and intervention of Veterans.
  • Function at the full extent of LPN's clinical privileges and scopes of practice.
  • Participate in team performance improvements and sustainment activities.
  • Implement primary care operations management processes as appropriate.
  • Manage communications and facilitate safe transitions of Veterans between care settings.
  • Provide health education and coaching on wellness and chronic disease management.
  • Engage Veterans and their families in health care processes and decision-making.
  • Ensure timely placement of Veterans in examination rooms and manage clinic workflow.
  • Maintain knowledge of telehealth technologies and assist in telemedicine operations.
  • Prepare the clinical environment for examinations and treatments.
  • Perform specimen collection and ensure proper handling and transport of specimens.
