About The Position

We are seeking Licensed Practical Nurse or Registered Nurse to work one-on-one with our pediatric client, in home and school setting, in the Ambler, PA area. The available shifts are Schedule: Monday through Friday 8a to 5p (45 hours/week) School Days: The nurse will assist the client in the morning preparing them for school and implementing the client's care plan, accompany the client on the bus and throughout the school day while monitoring for seizure activity. The nurse will ride home with the client after school around 3:45 and assist with after school care and monitoring at the client's home until 5pm. Non-school Days and/or Holidays: Services will be provided to the client in home from 8a to 5p. Additional Clients in surrounding area (Pottstown, Malvern, Collegeville, Phoenixville, King of Prussia, Skippack, Schwenksville, Royersford, Malvern, Phoenixville, Norristown, Lansdale, etc.) Full-time or Part-time available! Requirements

  • Current Pennsylvania Skilled Nursing License
  • Current CPR
  • Must be able to lift at least 50 pounds
  • Must pass State and Federal Criminal Background Checks
  • Must have up-to-date Physical and Two-Step PPD or Chest X-ray
  • Must be able to pass Skilled Nursing pre-employment test
  • Form of Transportation to Client's Residence

Responsibilities

  • Assess patient’s need for safety measures and appropriate interventions.
  • Implementation of the Nursing Care Plan as outlined by the Primary Nurse.
  • Implement medication changes according to verbal and/or written physician orders.
  • Administer prescribed medications and treatments.
  • Accurately observe and promptly report changes in the patient’s physical or mental status to the Nursing supervisor.
  • Accurately record nursing observations, activities, and treatments on the appropriate forms.
  • Ability to follow a physician's plan of care under the supervision of our Director of Nursing
  • Communicate with family members, in office staff and all supporting health professionals
  • Must be a caring and reliable individual who has the ability to work in a diverse atmosphere
