Requirements
- Current and active LPN license in good standing.

Responsibilities
- Collaborate in dynamic team huddles to promote seamless communication.
- Obtain comprehensive health histories and vital signs; prepare patients and assist providers with comfort and professionalism during exams and procedures.
- Collect and document critical health care data, both in person and over the phone.
- Support mental wellness by reviewing and entering PHQ-9 questionnaires.
- Provide age-appropriate, culturally sensitive nursing care guided by compassion and best practices.
- Serve as a patient advocate, reliable resource, and key contact throughout their care journey.
- Ensure accurate documentation of subjective and objective data in the EMR system.
- Administer medications and assist in proactive medication management.
- Perform lab and diagnostic testing with precision and urgency, reporting critical results promptly.
- Coordinate referrals and follow-up appointments with efficiency and empathy.
- Facilitate coordinated, high-quality care in partnership with physicians, nurses, and the broader health care team.
- Uphold strict confidentiality standards in line with HIPAA and infection control protocols.
- Contribute to a collaborative, respectful team environment, promoting open communication and effective problem-solving.
- Maintain universal precautions and safety standards for patient and staff well-being.
