This vital role involves utilizing community outreach to provide a range of healthcare education and services to individuals in need, with particular emphasis on wound management. Other key work will include linking clients to substance use disorder treatment, support for infectious disease management, and other public health assistance. As a Licensed Practical Nurse with our staffing division, here’s a big-picture view of what you’ll do: Work with a team of RNs, OTs, PTs, SLP's and HHAs to deliver quality patient care Take patient vitals, do assessments, document progress and report changes to RN supervisor Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques Educate family on the patient’s plan of care, medication and safety Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
