About The Position

"Navitas Healthcare, LLC" is seeking a Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. This evening shift role focuses on ensuring resident comfort, safety, and well-being while supporting the nursing team and maintaining facility standards. Only candidates within a 20-mile radius will be considered.

Requirements

• Active LPN license in New York
• Previous experience in long-term care or nursing home settings preferred
• Reliable availability for evening shifts
• Strong communication, interpersonal, and teamwork skills

• Ability to provide compassionate, patient-centered care

Responsibilities

• Deliver direct nursing care to residents according to care plans and facility policies
• Administer medications and treatments as prescribed
• Monitor resident conditions and report changes to supervising staff
• Assist residents with activities of daily living (ADLs), including hygiene, mobility, and feeding
• Document patient care accurately and in a timely manner
• Support nursing staff with routine clinical tasks and assessments
• Ensure compliance with infection control, safety, and facility regulations
• Maintain professional and compassionate communication with residents, families, and staff
