Location:

This position is based in Trinity, FL, serving patients in Zephyrhills, San Antonio, Dade City, Wesley Chapel and surrounding areas.

Your Role as a Licensed Practical Nurse (LPN):

Deliver high-quality nursing care to patients in their homes based on individualized care plans.

Whether you’re new to home health or an experienced professional, you’ll have access to the tools and support needed to excel.

You’ll also be part of a team that values collaboration and autonomy. While you’ll have the independence to manage your role, you’ll never be without the support of experienced clinical supervisors and a dedicated team focused on delivering exceptional care.

Requirements

• Unencumbered Florida LPN License (required).
• Valid driver’s license, auto insurance, and reliable transportation (required).
• IV Certification (required).
• Strong assessment and care planning skills.

Nice To Haves

• Experience in acute care or hospital settings (preferred).

Responsibilities

• Acute and chronic disease management.
• Medication management and administration.
• Assessing patient conditions and reporting changes.
• Developing tailored, goal-oriented treatment plans.
• Post-surgical rehabilitation.
• Educating patients and caregivers to support recovery and independence.
• Pain management and wound care.
