Requirements

Licensed Practical Nurse (LPN)) must hold a current unencumbered Nursing license to practice in the state of Georgia.
Current BLS Certification.
Current Driver’s License
Current TB Screening
Current BLS Certification.
Experience caring for medically fragile individuals.

Nice To Haves

Experience in Home Health (preferred).
Experience in Pediatrics (preferred).
Working knowledge of the policies and procedures of the Georgia Pediatric Program, GAPP, (preferred).

Responsibilities

Supervisory Visits and Education in a Patient's Home.
Re-evaluation of patients’ skilled nursing/PSS needs
Work autonomously with minimal assistance by the Clinical Manager
Maintain patient confidentiality
Make decisions based on established policies and procedures
Preparation of the clinical progress notes.
Informing the family and Registered Nurse of changes in the patients’ condition or needs.
Administration of medications and treatments as prescribed by a physician in accordance with currently accepted standards of medical practice
Total patient care
Patient and family teaching.
Other services in accordance with and as outlined in the Georgia Licensed Professional Nurse Practice Act.
Reports directly to the Registered Nurse and Clinical Director.
