Requirements

  • Graduate of an accredited practical nursing program and active, unrestricted Virginia LPN license
  • Minimum of 1,200 hours of LPN experience (home health preferred)
  • CPR certification, TB baseline screening
  • Strong clinical judgment, communication skills and a compassionate approach to care

Nice To Haves

  • Experience with feeding tubes preferred, ability to recognize and manage seizures

Responsibilities

  • Deliver high-impact, one-to-one skilled nursing care aligned with physician orders and individualized care plans
  • Provide specialized clinical support, including management of enteral feeding tubes (G-tube/PEG), medication administration, and treatments
  • Be the eyes and ears of patient care - proactively monitor condition, identify changes, and act quickly to ensure the best outcomes
  • Create continuity and trust by building strong relationships with patients and their families
  • Own the quality of care delivered through accurate, timely documentation and clear communication with the care team
  • Advocate for your patient's well-being, ensuring comfort, safety, and dignity in every interaction
