Requirements

High school diploma or GED
Active, LPN license in the state of Illinois
Compassionate, dependable, and strong communication skills
Ability to maintain resident confidentiality
Ability to lift, move, and assist residents safely
Must pass a criminal background check and required health screenings (TB and others)
Ability to read, write, and understand English

Nice To Haves

Supervisory experience preferred
Experience in long-term care or a similar setting preferred

Responsibilities

Provide safe, compassionate nursing care to residents based on individualized care plans
Administer medications and treatments accurately
Monitor residents’ health and report changes to the RN, provider, and families as needed
Document assessments, care, and observations in the EHR
Support and guide caregivers and CNAs during daily care
Communicate effectively with physicians, families, and the care team
Participate in care plan meetings, quality initiatives, and state surveys
Follow infection control and safety procedures
Complete other duties as assigned
