We offer customized, individualized rehabilitation tailored to the specific needs of those recovering from a stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury. A Licensed Practical Nurse (LPN) who excels in this role: The Licensed Practical Nurse at St. Mary Rehabilitation Hospital plays an essential role within a collaborative and patient-centered environment. As a 50-bed inpatient rehabilitation hospital jointly operated by Trinity Health Mid-Atlantic and Lifepoint Rehabilitation, we specialize in helping patients recover from stroke, brain injury, neurological disorders, orthopedic trauma, and other complex conditions. Our interdisciplinary team approach, combined with modern facilities and a dedication to quality, allows us to provide exceptional rehabilitative care that transforms lives.

Requirements

  • Applicants should have a current Pennsylvania License as a Practical/Vocational Nurse, and have graduated from an accredited licensed practical/vocational nurse program.
  • CPR/BCLS certification required
  • IV Certification required or obtained within 6 months (highly preferred)
  • Minimum six months’ Medical/Surgical experience in an acute care setting preferred
  • Excellent oral and written communication and interpersonal skills

Nice To Haves

  • ACLS preferred
  • IV Certification required or obtained within 6 months (highly preferred)
  • Minimum six months’ Medical/Surgical experience in an acute care setting preferred

Responsibilities

  • Deliver direct patient care under the supervision of a Registered Nurse, in alignment with individualized care plans
  • Monitor patient conditions and report changes promptly to nursing and clinical teams
  • Administer medications and treatments as prescribed and within LPN scope of practice
  • Assist patients with activities of daily living and promote mobility and independence
  • Maintain a clean, safe, and supportive patient environment
  • Document all care activities, observations, and patient responses accurately and in a timely manner
  • Support the interdisciplinary team by contributing to quality improvement and patient safety initiatives
  • Communicate effectively with patients, families, and team members to support care planning and discharge readiness
  • Provide compassionate, professional, and proactive care in an inpatient rehab setting
