Requirements
- Active Licensed Practical Nurse (LPN) license in Ohio (required)
- CPR certification required
- Experience in skilled nursing, rehabilitation, or long-term care preferred
- Strong assessment, problem-solving, and communication skills
- Compassionate, dependable, and team-oriented

Responsibilities
- Deliver high-quality, resident-centered nursing care according to individualized care plans
- Safely administer medications and treatments with clinical accuracy
- Monitor residents health conditions and communicate changes promptly to RNs and physicians
- Collaborate with interdisciplinary teams including therapists, social services, and families
- Provide guidance and support to CNAs, ensuring high standards of care delivery
- Maintain a safe, clean, and supportive environment for residents and staff
