Licensed Practical Nurse (LPN)
About The Position
Affirmed Home Care is a premier, concierge-level home health agency committed to delivering exceptional, personalized care with compassion, dignity, and integrity. We are currently seeking Per Diem Licensed Practical Nurses (LPNs) to provide high-quality, one-on-one care to clients in the comfort of their homes throughout Bergen County. We offer a wide range of opportunities, including 6-, 8-, and 12-hour shifts, day, night, and overnight cases, as well as visit-based assignments. This role is ideal for nurses seeking flexibility, consistent support, and meaningful work outside of a facility setting.
Requirements
- Minimum 1 year of recent LPN experience (acute care required; home care preferred)
- Active New Jersey LPN license and current BLS certification
- Authorized to work in the United States
- Current physical exam (within the past 12 months)
- PPD or QuantiFERON (within 12 months) or chest X-ray (within 5 years)
- MMR immunizations (within 10 years)
- Flu vaccination or approved medical exemption
