About The Position

The Center for Wound Healing & Hyperbaric Medicine, LLC (CFWH) provides management and consulting services to healthcare providers in the area of wound care and hyperbaric medicine. By focusing on the needs of the patients as well as the partners we serve, CFWH is able to bring advanced wound healing strategies to many communities around the country. The Licensed Practical Nurse (LPN) will provide clinical care within their scope of practice related to wound care and/or hyperbaric oxygen therapy. This position involves case management and administrative duties, performing a combination of nursing and technician responsibilities. The LPN will act within the guidelines of federal and state laws, Joint Commission or DNV ISO 9001 Standards, and professional ethical practices.

Requirements

Current state Licensed Practical Nurse license, graduated from an accredited school.
Current Basic Life Support (BLS) from approved agency.
2+ years of clinical nursing experience, preferably hospital or outpatient clinic.
1+ years of experience in wound care required.
Strong verbal and written communication skills.
Ability to treat others with compassion and patience.
Ability to work in a fast-paced team-oriented environment.
Must possess excellent interpersonal skills.

Nice To Haves

Proficiency with Microsoft 365 tools, particularly SharePoint preferred.

Responsibilities

Under direction and supervision, performs patient care, including, but not limited to: taking vitals, documenting in an electronic medical record (EMR), measuring and photographing wounds, and sending or faxing out wound care supply / home care orders.
Assists physicians and RNs with wound care procedures within scope of practice.
Is responsible for cleaning and stocking of patient care areas which includes putting away of supplies, infection control and maintaining a clean work environment.
Follows department policies and procedures as it pertains to items approved in a wound care and hyperbaric environment to ensure the safety of our patients. Communicates any findings to the immediate supervisor / HBO safety director before any treatment commences.
Informs management of any changes in working conditions and/or concerns, such as infection control, hazardous material control, confined spaces, etc.
Proficient in emergency fire safety procedures within the wound center and hyperbaric environment.
Under direction and supervision, provides patient education as it pertains to wound care dressings, hygiene, etc. and/or hyperbaric oxygen therapy.
Provides hyperbaric oxygen therapy to patients for approved acute/chronic conditions.
Responsible for thorough and accurate documentation in the Electronic Medical Records (EMR) system in timely manner according to organizational policies, procedures and guidelines.
Assists in answering phones, coordinating patient scheduling, obtaining new patient phone intakes, and obtaining additional clinical documentation.
Conducts nursing assessments (within scope of practice) and thoroughly documents in electronic medical record (EMR) health status by collecting objective and subjective data from observations, exams, interviews and written records; sorting, selecting, reporting and recording the data; validating, refining, and modifying the data. Bases the scope and depth of assessment on the patient’s immediate condition or need.
In collaboration with the patient, RN, and Physician, develops a plan of care based on nursing assessment and diagnosis that prescribes interventions to attain expected outcomes. This includes identifying the holistic needs of the patient, treatment decisions and priorities.
Identifies community resources needed for continued care and makes referrals.
Implements the plan of care and the nursing interventions for the patient.
Explains plan of care to the patient in a way that is understandable and educates the patient thoroughly.
Thoroughly documents in electronic medical record (EMR) patient information in an accurate and timely manner, according to organizational policies, procedures and guidelines.
Consistently adheres to company, hospital, and accrediting body clinical, safety, and compliance standards.
Ensures compliance with the Health Insurance Portability and Accountability Act (HIPAA) by safeguarding patient information and maintaining the highest level of confidentiality standards.
May be responsible for participating in the onboarding and training of new team members.
Transports patients to and from appointments if service is offered at your work location.
