About The Position

A Licensed Practical Nurse (LPN) at Merakey provides essential clinical support and medication management to individuals with intellectual and developmental disabilities or behavioral health needs. This role focuses on maintaining the health and safety of individuals while working under the direction of a Registered Nurse or physician.   Job Summary The LPN is responsible for delivering direct nursing care and monitoring the physical and mental well-being of individuals in residential or community settings. This position involves executing medical orders, performing routine clinical tasks, and ensuring that all health-related interventions are documented accurately and in accordance with state regulations.

Requirements

  • A valid and unencumbered Licensed Practical Nurse license in the state of employment.
  • Graduate of an accredited Practical Nursing program.
  • Current CPR and First Aid certification.
  • Strong observational skills and the ability to respond calmly in emergency situations.
  • A valid driver’s license and reliable transportation for travel between service sites if required.
  • Successful completion of all required criminal background checks and health screenings.

Nice To Haves

  • Previous experience in a behavioral health or intellectual disabilities setting is highly desirable.

Responsibilities

  • Administering prescribed medications and treatments to individuals according to physician orders.
  • Monitoring for side effects, adverse reactions, or changes in medical status and reporting findings to the RN or physician.
  • Assisting in the development and implementation of individual health care plans.
  • Conducting routine health screenings, including monitoring vital signs, blood sugar levels, and weight.
  • Providing wound care, tube feedings, and other specialized nursing procedures as required by individual needs.
  • Collaborating with the interdisciplinary team to ensure medical needs are integrated into the individual’s daily support plan.
  • Training and supervising non-licensed staff on medication administration and health-related protocols.
  • Maintaining thorough and accurate medical records, including medication administration records and nursing progress notes.
  • Assisting with medical appointments and coordinating follow-up care with external healthcare providers.
