As a Full-Time Licensed Practical Nurse (LPN) at VNA of Ohio Hospice Operations, LLC, your day-to-day responsibilities will center around providing exceptional patient care in a supportive, onsite environment. You will assist registered nurses in developing individualized care plans, monitor patients' vital signs, and administer medications as prescribed. Daily tasks will include documenting patient progress, collaborating with a multidisciplinary team, and effectively communicating with families regarding care plans and patient needs. You will also be responsible for providing basic patient care activities such as bathing, dressing, and mobility assistance. Ensuring safety and comfort for each patient will be paramount in your role, requiring attention to detail and adherence to established protocols. Requirements

  • Current LPN license in Ohio
  • Completed an accredited nursing program
  • A minimum of 1-2 years of clinical nursing experience
  • Deep commitment to patient-centered care, ensuring the dignity and comfort of patients during end-of-life care
  • Strong observation and reporting skills
  • Proven team collaboration
  • Excellent communication skills
  • Good understanding of hospice regulations and compliance standards
  • Comfortable using electronic medical records (EMR) systems
