About The Position

ABA Homecare, located in Conyers, GA, is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality home health care services to patients in Rockdale, Newton, Henry, Gwinnett, and Fulton counties . Position Summary The LPN will provide skilled nursing care in the home setting, support patient health goals, and work collaboratively with the care team to ensure safe, consistent, and patient-centered care. This role is ideal for a nurse who values independence, flexibility, and making a meaningful difference in the community.

Requirements

• Current and active LPN license in the State of Georgia.
• Current CPR & First Aid certification.
• At least 1 year of nursing experience preferred, home health experience a plus.
• Strong clinical judgment, communication, and time management skills.
• Reliable transportation and willingness to travel within assigned counties.
• Ability to work independently in a home care setting, as well as with a multidisciplinary team.
• Maintain open line of communication with team members, case management, and other support staff

Responsibilities

• Provide direct nursing care to patients in their homes.
• Monitor patient condition and report changes to the supervising nurse or care team.
• Provide support to caregivers and patients in the community, conducting regular site visits.
• Administer medications and treatments as ordered and within scope of practice.
• Assist with wound care, vital signs, and routine assessments.
• Document care accurately and in a timely manner.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Follow all agency policies, procedures, and applicable state and federal regulations.
• Promote patient safety, comfort, dignity, and quality of care.
