About The Position

At Frederick Living, the LPN provides skilled nursing care to residents while leading the neighborhood team. This role promotes Person-Centered Care, supervises daily resident care, and supports the professional growth of staff.

Requirements

Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania
High school diploma or equivalent
BLS Healthcare Provider certification
Basic computer skills
Strong communication skills, both verbal and written
Ability to role model professional appearance and demeanor
Demonstrates respect, integrity, compassion, and excellence
Commitment to person-centered care initiatives
Conflict resolution skills and ability to coach and support staff
Flexible schedule and willingness to work extra hours as needed
May be asked to assist in emergencies and resident evacuations

Nice To Haves

Prior experience in geriatric nursing preferred

Responsibilities

Observe and report changes in resident condition to the Nurse Supervisor or physician/physician extender
Communicate with Supervisor, ADON, DON, NHA, care providers, and family/POA as needed
Use evidence-based practices to ensure safe, high-quality care delivery
Assist in managing personnel through performance evaluation, Care Chats, coaching, counseling, education, and corrective action as needed
Complete all required documentation accurately and in a timely manner
Participate in resident transitions of care, care conferences, and care plan reviews
Collaborate with Cedarwood staff to ensure optimal resident outcomes
Comply with Infection Control practices
Ensure hand-held communication equipment is maintained for effective call bell management and team communication
Attend and complete all required in-services, training, and team meetings
Abide by the organization’s code of conduct
Perform other duties as assigned
